Frequently Asked Questions About Hospice Care
Proactive Hospice provides compassionate home hospice care for families in Macomb, Oakland, and Wayne counties. Here are some of the most common questions we receive to help you understand what hospice is and how it can help.
What is hospice care?
Hospice care is specialized care for individuals facing a terminal illness, generally with a life expectancy of six months or less if the illness follows its normal course. Hospice focuses on comfort, dignity, symptom management, and quality of life rather than curative treatment. Our hospice team supports patients and their families physically, emotionally, and spiritually while helping patients remain as comfortable as possible wherever they call home.
Who is eligible for hospice services?
A patient may be eligible for hospice care when they have a terminal illness and a physician certifies that their life expectancy is generally six months or less if the illness follows its normal course. Hospice is appropriate when the focus of care shifts toward comfort, symptom management, dignity, and quality of life rather than treatment intended to cure the terminal illness. Our hospice team can also help patients and families understand eligibility and determine whether hospice care is appropriate.
What is the cost of hospice care?
Hospice care is covered by Medicare for eligible beneficiaries and may also be covered by Medicaid and many private insurance plans. Depending on the patient’s coverage, hospice benefits may include visits from the hospice care team, medications related to the terminal illness, medical equipment and supplies, and other covered hospice services. Proactive Hospice Care can help verify your insurance benefits and explain any potential costs before services begin.
Where can hospice care be provided?
Hospice care can be provided wherever the patient calls home. This may include a private residence, a family member’s home, an assisted living facility, a nursing facility, or another appropriate care setting. Our hospice team provides scheduled visits and coordinates care with the patient, family, caregivers, and facility staff when applicable, helping patients remain in a familiar and comfortable environment.
Does hospice mean giving up hope?
Absolutely not. Hospice does not mean giving up hope. Instead, the focus shifts toward comfort, quality of life, dignity, and making the most of each day. Our hospice team supports patients and families physically, emotionally, and spiritually, helping them find comfort, peace, and meaningful moments throughout the hospice journey.
How do I start hospice services with Proactive Hospice?
Getting started is simple. You can contact Proactive Hospice Care directly, and our team will guide you and your family through each step. We will coordinate with the patient’s physician, review the patient’s needs, and help determine hospice eligibility. Once hospice care is elected and the necessary documentation is completed, our team will develop an individualized plan of care and coordinate services as quickly as possible.
What kind of care and services does the hospice team provide?
Our interdisciplinary hospice team provides a wide range of services based on each patient’s individualized plan of care, including:
• Pain and symptom management by hospice nurses and physicians.
• Personal care assistance from hospice aides.
• Emotional, psychosocial, and spiritual support from social workers and chaplains.
• Medications related to the terminal illness and related conditions, as covered under the hospice benefit.
• Medical equipment and supplies related to the hospice plan of care.
• Volunteer companionship and support when appropriate.
• Bereavement support for families following the loss of a loved one.
• 24/7 on-call availability for questions, concerns, and changes in the patient’s condition.
How long can a patient receive hospice care?
A patient can receive hospice care for as long as a physician certifies that they have a life expectancy of six months or less. If a patient lives longer than six months, a physician can re-certify them to continue receiving care, as long as they continue to meet the eligibility criteria.
